Chronic Disease & GP Managed Care Plans
Patients living with chronic health conditions such as diabetes, hypertension, osteoporosis, arthritis, and chronic pain can be further supported with the implementation of a GP managed care plan. We provide comprehensive and individualized care plans that ensure all aspects of our patients’ chronic health conditions are identified and managed. Our GPs also work closely with our patients to develop and work towards achievable goals concerning their chronic health conditions. Some care plan patients are eligible for referral to allied health providers at a subsidized cost.
