As the ageing population in Australia continues to grow, so too does the complexity of the management of chronic health issues. As such, our GPs are skilled in providing appropriate primary care to those living in residential aged care settings. We work collaboratively with other healthcare providers and the resident’s family to deliver high quality primary care to older people living in aged care facilities.

Process
First, we like to meet with the resident and/or their Enduring Power of Attorney (EPA) at our clinic, in order to get to know each other and discuss the services we offer. This is also an opportunity for our team to review the resident’s existing medical records and current medication list.
Next, we visit the resident at the aged care facility. Here, we complete a comprehensive physical examination and organise a medical management plan, which is regularly reviewed based on the resident’s health condition.
We then provide ongoing medical care for the resident within the care home as required, between the hours of 9am-5pm. We recommend either the Canberra After Hours Locum Medical Service (CALMS) or the National Home Doctor Service (NHDS) for after hours medical concerns.
We hold case conferences with the resident and/or their EPA, as well other providers involved in the resident’s care, every 3-6 months. During these conferences, we discuss the overall care of the resident, including their health status and current medications, using a multidisciplinary approach. These conferences are designed to keep everyone involved in the resident’s care on the same page, in order to achieve the best outcomes for the resident and their family.
In this journey, we work closely with you and the aged care facility to address the needs of the resident and best manage their health issues, which improves the overall quality of life of the resident.
Costs
Many people living within residential aged care facilities have complex health problems that may require more frequent visits to the care home. Therefore, our professional attendance, consultations and other services provided to our aged care residents are covered by Medicare/DVA with no additional cost to the patient.
