Patients living with chronic health conditions such as diabetes, hypertension, osteoporosis, arthritis, and chronic pain can be further supported with the implementation of a GP managed care plan. We provide comprehensive and individualized care plans that ensure all aspects of our patients’ chronic health conditions are identified and managed. Our GPs also work closely with our patients to develop and work towards achievable goals concerning their chronic health conditions. Some care plan patients are eligible for referral to allied health providers at a subsidized cost.

A GP-managed care plan helps patients better understand and manage their conditions, supports ongoing monitoring, and ensures they receive the right care from the right providers at the right time.

What is a GP Management Plan (GPMP)?
A GPMP is a personalised plan created by your GP that outlines your chronic condition, treatment strategies, health goals, and supports needed to manage your condition effectively.
Am I eligible for subsidised allied health visits?
Patients with a chronic condition lasting longer than six months may be eligible for Medicare-subsidised visits to allied health providers through Team Care Arrangements.
How often should my care plan be reviewed?
Care plans are typically reviewed every 12 months, but your GP may recommend more frequent reviews depending on your condition.
Do chronic disease care plans cost extra?
Many care plan services are covered under Medicare, meaning there may be little to no cost for eligible patients. Your GP can advise you about potential fees.
What conditions qualify as chronic?
Any condition lasting longer than six months may be considered chronic, including diabetes, arthritis, asthma, chronic pain, heart disease, mental health conditions, and more.
